All three self-screeners – the Student Self-Screener, the Employee Self-Screener, and the Visitor Self-Screener – are accessible from the district’s COVID-19 protocols page and are posted on each school web page.

Parent Involvement Information

Fivay High School has started the ABC Program (Assist, Believe, Care) to provide support to students and their families if they are experiencing economic hardship and struggle with acquiring clothing or supplies for school. If your family or student needs assistance in any of these areas OR you wish to donate, please complete and return this form to the Fivay ABC Coordinator Susan Libby, Slibby@pasco.k12.fl.us, or email, or call extension 727 246 -4000 and ask for Mrs. Libby.

Interested in Volunteering? 

Thank you for your interest in volunteering with Pasco County Schools. Volunteers are a vital part of building relationships between schools, parents, community members, and business partners, while adding value to the educational experience of Pasco County students. Anyone wishing to volunteer at a Pasco County school must complete a volunteer application. Florida law requires all school volunteers to undergo a new background check each year.Currently volunteer opportunities will be limited and will be closely monitored to ensure mandated policies are followed. As always, only approved volunteers will be able to assist schools and opportunities will be on an as needed basis. After filling out the volunteer form, we ask that you receive approval from the school before arriving to campus to volunteer.

School Advisory Committee

The School Advisory Committee (SAC) is a school-based group intending to represent the school, the community, and the people closest to the students.  The group shares responsibility for guiding the school toward continuous improvement.  It is responsible for final decision-making at the school relating to implementation of provision of the annual School Improvement Plan.  It is composed of the principal and teachers, education support employees and students, parents, and other business and community citizens.

Fivay Partner Opportunities

Do you have a business, or patronize a business that might consider donating to our student and teacher incentives program?

Have your business’s banner prominently displayed in our drive-through loop.  Your business can be patronized by our 1,700 families and 170 teachers and staff by Advertising at Fivay. Thousands of cars drive through our school on a weekly basis and will see your banner in the pick-up/drop-off loop.  If our 1,700 families and 170 staff weren’t already patrons or fans of your business, they certainly will be now!

We will use the proceeds to reward outstanding students and staff.  Our student recognition team will provide incentives and rewards to show our students and staff how much their hard work is appreciated.  Our goal is to reward students that are on track for a diploma, on the honor roll, and students that have overcome barriers to getting their diploma. We would also like to reward our teachers for their hard work.
